Alpha-1 antitrypsin deficiency does not always lead to emphysema even when the patient has 1 or 2 Z alleles (which are more specific for the pulmonary phenotype).  As long as serum levels of A-1 AT are > 10, enough should get to the lung parenchyma to prevent excessive elastase damage to alveoli.
