If one of your NASH patients, who typically has AST<ALT, develops a LFT panel where AST>ALT, you should worry that they’ve progressed to cirrhosis.  This would be particulary true if they also had obesity, diabetes and advanced age, three recognized risk factors for advanced stages of fibrosis.
