You need to palpate for splenomegaly but an obese abdomen is prohibitive. Try percussing Traube’s space: identify the left anterior, mid-thoracic rib spaces and tap away. Remember, the enlarging spleen grows cephalad as well as caudad, and only two things should cause dullness in this space, a spleen or a post-prandial stomach.
