Definition: orthodeoxia

Orthodeoxia is a physical sign of AV shunting that occurs in the setting of hepatopulmonary syndrome (HPS); the patient feels dyspneic when sitting up versus supine.  In HPS, there is extensive pulmonary arteriolar vasodilation at the lung bases, so when in a dependent position, more blood flows to this region and poor gas exchange ensues.
