Clinical Pearl

You are reviewing a staging liver biopsy, and there is at least a moderate pretest probability of cirrhosis.  The biopsy specimen is fragmented, making it difficult to visualize in-tact nodules.  The diagnosis of cirrhosis is uncertain…or is it? A cirrhotic liver is well known to fragment during H&E preparation.  That might be the best clue of all.
