Definition: HBV precore mutant

Hepatitis B Virus precore mutant (sAg+, sAb-, eAg-, eAb+, cAb+) arises in the setting of longstanding infection, after the wild type eAg positive disease seroconverts to eAb positive disease and the initial infection is cleared.  A mutated virus emerges, replicates at lower levels and should be treated when ALT is abnormal and viral load is >2,000 IU/mL.
